Ordering Guidance


The recommended first-tier test to screen for Smith Lemli Opitz (SLO) is measurement of cholesterol precursors in plasma. Order SLO / Smith-Lemli-Opitz Screen, Plasma.

Specimen Required


Patient Preparation: A previous bone marrow transplant from an allogenic donor will interfere with testing. For instructions for testing patients who have received a bone marrow transplant, call 800-533-1710.

 

Submit only 1 of the following specimens:

 

Specimen Type: Whole blood

Container/Tube: Lavender top (EDTA) or yellow top (ACD)

1. Invert several times to mix blood.

2. Send whole blood specimen in original tube. Do not aliquot.

Container/Tube: Sterile container with any standard cell culture media (eg, minimal essential media, RPMI 1640). The solution should be supplemented with 1% penicillin and streptomycin.

Collection Instructions:

1. An alternative blood collection option for a patient older than 1 year is a fingerstick. For detailed instructions, see How to Collect Dried Blood Spot Samples.

2. Let blood dry on the filter paper at ambient temperature in a horizontal position for a minimum of 3 hours.

3. Do not expose specimen to heat or direct sunlight.

4. Do not stack wet specimens.

5. Keep specimen dry

Clinical Information

Cholesterol plays an essential role in many cellular and developmental processes. In addition to its role as a membrane lipid, it is the precursor to numerous molecules that play important roles in cell growth and differentiation, protein glycosylation, and signaling pathways. The biosynthesis of cholesterol and its subsequent conversion to other essential compounds is complex, involving a number of intermediates and enzymes. Disorders that result from a deficiency of these enzymes lead to an accumulation of specific intermediates and inhibit the formation of important biomolecules. Clinical findings common to cholesterol biosynthesis disorders include congenital skeletal malformations, dysmorphic facial features, psychomotor retardation, and failure to thrive.

 

Smith-Lemli-Opitz syndrome (SLO) is an autosomal recessive disorder caused by alterations in the DHCR7 gene leading to a deficiency of the 7-dehydrocholesterol reductase enzyme. It is characterized biochemically by markedly increased plasma concentrations of 7-dehydrocholesterol (7-DHC) and 8-dehydrocholesterol (8-DHC) levels. Clinically, features can include microcephaly, growth retardation, developmental delay, dysmorphic facial features, cleft palate, limb abnormalities (especially 2-3 syndactyly of the toes and postaxial polydactyly), and heart and kidney malformations. The clinical spectrum ranges from mild to severe, with some mildly affected individuals presenting with only 2 to 3 toe syndactyly and mild cognitive impairment.

 

SLO is inherited in an autosomal recessive manner, caused by disease-causing variants in both alleles of the DHCR7 gene. The reported incidence is from 1 in 20,000 to 1 in 40,000, but it may be more prevalent due to underdiagnosis of mildly affected individuals.

 

Measurement of cholesterol precursors in plasma (SLO / Smith-Lemli-Opitz Screen, Plasma) should be performed as a first-tier test to screen for SLO.

 

Cholesterol supplementation may result in clinical improvement but is not curative.
